This Certificate entitles one child to any one of the above Sport&Health Kids Class 6-week sessions.  All classes start the week of February 13th.

Upon checkout, please select your class choice.

IMPORTANT:  After purchasing your class(es) on Certifikid, please fill out THIS FORM for each Certificate purchased.  This will constitute your registration for the class.  Once you fill out this form, all you have to do is show up on the first day of class!
